Instructions for Leaves Requests

Please read the applicable policies below before proceeding:

For Professional Leave, Vacation, please complete the "Request for Leave” form given to you at resident orientation. You can request the form from our Department (Dept. of Family Medicine, 2nd Floor, Clinical Skills Bldg., The University of Western Ontario, London ON N6A 5C1, 519-661-2037)

For Maternity/Parental/Health/Compassionate Leave, please follow these instructions: 

• Email the postgraduate director (FMPGD@schulich.uwo.ca) with the leave request with the following information: 
• Your full name and PGY year
• The type of leave you intend to take
• The start and end date of your leave
• Description of the situation requiring your leave (if applicable)
• The postgraduate director will get back to you via email ASAP
